Only authorized users are permitted to submit a Service Request. Please contact your IT Coordinator first to discuss your request.
If you are an authorized user, go to Port Request. Use this page to request port activations, port deactivations, or port repairs. Most requests will have an associated service charge. Please note that the Port Request page is temporarily linked back to our previous web site design where some of the links may not be fully functional.
Tips for requesting ports:
- Choose the correct department from the pull-down list. Choosing the wrong department may cause billing problems and delays.
- If you believe that this is a warranty repair, please state that in your request.
- Be precise in wording your request. Use the words “activate” or “deactivate”. Give the exact room number and building name or number. Write the information from the existing wall plate label. Include a contact name and phone number. How many ports do you expect to be active when the change is completed? If the room is not allocated to the correct department or there are special billing requests, please enter the PeopleSoft account code that should be used for billing.
